Our STOCK Index Chart Dividers are available in over 50 selected titles using 16 standard tab colors to organize your patient charts to meet the needs of your particular practice. Made from 90 lb. stock, all indexes have tabs printed on both sides, Mylar covered and reinforced holes opposite tabs. Each tab title is packaged 100 indexes per shrink wrapped package. Tabs are 1/6 cut (1-11/16" wide). 1/2" extension. Bottom tab style has 2 holes punched @ top. Side tab style has 2 holes punched @ top and side.
